MyMedicare is a voluntary program that formalizes your relationship with your GP and primary care team, aiming to enhance your healthcare.

Tailored Care and Services:

• Improved Health Outcomes: Formalized relationships with your GP.
• Extended Telehealth: Longer consultations funded by Medicare, especially for children, concession card holders, mental health, and disability support.
• Enhanced Aged Care: More regular GP visits and better care planning for residents from August 2024.
• Better Hospital Discharge: Improved care coordination for frequent hospital visitors from mid-2024.
